Information for Parents and Students
Congratulations on your selection to attend the 2012 Missouri Leadership Seminar (MLS)!
The 2012 MLS will take place June 8-10, 2012 at the University of Central Missouri in
Warrensburg. MLS is offered free of cost to the student, parents, and school! The only
cost that we cannot cover is transportation. Students are sponsored by various community
organizations and businesses throughout the state.

As you prepare to attend MLS, please make note of these important dates:
*  April 1, 2012: Registration Deadline
*  June 8, 2012: On-Site MLS Check-In & Registration
*  June 10, 2012: Parent Orientation Session & Closing Ceremony
To complete the Registration Process, please follow these steps:
1. Complete the online Registration Form:
*  The Registration Form must be completed in its entirety prior to submitting.
*  You will not be able to save your information and come back to the form.
*  The Registration Form must be electronically signed by both the student and a parent/guardian.
Student registrations must be completed by April 1st, 2012.

2. Submit a copy of your Health Insurance Card:
You must submit a copy of the FRONT and BACK of your health insurance card. Please include the student’s name if it is not listed on the card.

3. Confirmation of Registration
You will receive a confirmation email when your Registration is complete and has
been reviewed by our staff. We will also contact you if we are missing any of your
information.
